Responsibilities

  • Provide high-quality nursing care to patients in their homes and community settings.
  • Assess patient needs and develop individualized care plans in collaboration with healthcare professionals.
  • Administer medications and perform clinical procedures as necessary.
  • Educate patients and their families about health management and self-care techniques.
  • Monitor and evaluate patient progress and adjust care plans accordingly.
  • Maintain accurate and timely documentation of patient care activities and assessments.
  • Serve as a liaison between patients, families, and other healthcare providers to coordinate care.
  • Participate in community health initiatives and health promotion activities.

Requirements

  • Registered Nurse (RN) qualification with a valid nursing license.
  • Proven experience in a nursing role, preferably in community or district nursing.
  • Strong communication and interpersonal skills with the ability to build rapport.
  • Excellent organizational and time-management abilities.
  • Ability to work independently and as part of a multidisciplinary team.
  • Competency in using healthcare technology and electronic health records.
  • A valid driver’s license and ability to travel to various patient locations.
